The staff of Neurology Associates will be happy to answer any questions concerning fees for services or the insurance plans that are accepted. Insurance billing is provided by our office.

Questions regarding your coverage and benefits should be directed to your employer or insurance company prior to scheduling an appointment.

Payment for services not covered by your insurance is due at the time of your appointment. Your insurance company may require you to provide a co-pay at your visit.

Information to Bring to Your Appointment:

• Insurance card
• Physician referral forms if required by your insurance company
• List of current prescriptions and over the counter medicines-dose and frequency
• Pertinent health history including surgeries
• Recent test results/x-rays
• Patients under 18 years of age must be accompanied by a parent or legal guardian
